REQUEST FOR PROPOSAL

LOCAL & LONG DISTANCE TELEPHONE SERVICE

&/OR PAGING SERVICE

The Edwardsburg Public School is accepting bids for local and long distance telephone service for all of its buildings. This service is to begin July 1, 2003.

Presently there is a T-1 circuit and a 56k with 24 trunks plus 16 single-lines serving the Edwardsburg Public Schools connected to seven buildings. For long distance service please indicate the following:

· Per minute charge in Michigan (intralata)

· Per minute charge outside of Michigan (interlata)

All bidders agree to participate in the E-rate program of the Universal Service Fund. In addition they agree to comply with all of the rules and procedures required by the Schools and Libraries Corporation and the USF.

Your sealed bids attached to the enclosed bid form should be marked "USF - Local & Long Distance Service" and sent to the Edwardsburg Public Schools Business Office, 694 10 Section Street, Edwardsburg, MI 49112. Questions about local and long distance phone service may be directed to: Patricia Bellaire, Director of Business and Operations - (269) 663-3055, ext. 247 or (269) 663-1047 between 8:00 a.m. and 3:00 p.m. daily. All bids are to remain firm for 90 from the last day of the 28 day bidding procedure set up by the SLC.

We reserve the right to accept or reject any or all bids in whole or in part, to inspect any/or all bids, and to waive informalities, irregularities, and/or errors in the bidding to the extent permitted by law. Awards will be made in the best interest of the District.
